why this code is not working?

I noticed recently in Claude Code (2.0.76, Opus 4.5), the agent is more likely to reward hack / engage in adversarial behaviours:

- Writing tests that resembles real useful tests, but don't actually test anything.
- Skipping steps in implementation "autonomously" because it thinks it is reasonable
- Leave difficult tasks to the user because... it is difficult
- Assert certain assumptions about the project to reduce the scope of the task without properly checking it
