Podocyte gluttony: Incidental finding in 64 yo F with AKI. Super-constipated podocytes (Mega-Chang Bodies 😂) after Thanksgiving dinner, STUFFED with lipid inclusions. Puzzling, until patient said "Oh, I forgot to tell you I have lupus!". Plaquenil-associated phospholipidosis 🤦‍♀️

ALL: Never quite seen anything like this in a COVID patient and AKI. I was expecting just ATN. Instead saw a sea of blue cells separating tubules. Work-up revealed acute lymphoblastic lymphoma/leukemia (ALL). 35-yr old previously healthy man with recent history of COVID pneumonia

MBUO (Myeloid Bodies of Unknown Origin): 47 HF with malignant hypertension and acute renal failure. Bx with impressive vascular TMA changes and lots of these lipid inclusions ONLY in mesangial and endothelial cells (NOT in podocytes). No known drug/toxin exposure. 🤷‍♀️ @ldcornell

Touched by lupus: I don't know why some deposits seem to organize into these "fingerprint" structures more than others, but I've seen them in all stages and all locations... And the saying holds true, even for gloms: No one's fingerprints are exactly the same as anyone else's!

Today's Two-fer: 78 yo M with HTN, biopsied for worsening renal function. Surprise: light chain deposition disease AND light chain crystalline tubulopathy, both lambda associated. SPEP confirmed IgG-lambda spike.
