Heparin: A continuously running drip that takes up a line and requires aPTT/ Xa blood draws, order changes and dual sign off every 6 hours.

Hospitalists, if a patient is STABLE on elequis and can take pills WHY ARE YOU CHANGING IT TO HEPARIN

@pathlesstaken4 Maybe. Capacity to modulate Heparin also increases risk though.

My working theory is that it makes the hospital more money since Heparin is cheaper.
