In the pilot STOP-IMH randomised trial, immediate ticagrelor monotherapy after primary #PCI for #STEMI showed no apparent excess in 1-year ischaemic events while significantly reducing non-access site bleeding compared with #DAPT.

Does PCI complexity matter when selecting antiplatelet therapy after #PCI for #ACS?
This post hoc analysis of the NEO-MINDSET trial found that PCI complexity did not modify the effects of potent #P2Y12 inhibitor monotherapy versus #DAPT on ischaemic or bleeding outcomes. Monotherapy reduced clinically relevant bleeding, but without a clear net clinical benefit.

Can intraprocedural haemodynamics improve risk stratification after M-TEER?
In the OCEAN-Mitral registry, a reduction in mean left atrial pressure during M-TEER was independently associated with better long-term outcomes in degenerative MR, but not in functional MR, uggesting the potential value of real-time physiological assessment beyond MR reduction alone

Can ticagrelor monotherapy be considered after PCI in patients with chronic coronary syndrome at high ischaemic risk?
In this post hoc analysis of the TWILIGHT trial, ticagrelor monotherapy after 3 months of DAPT showed similar ischaemic and bleeding outcomes to continued ticagrelor-based DAPT in patients both with and without high ischaemic risk.

How should transvalvular gradients be assessed after TAVI?
This propensity score-matched study found that invasively measured gradients, but not echocardiography-derived gradients, independently predicted mortality, highlighting the potential prognostic value of invasive haemodynamic assessment after valve implantation. `

Does timing of ventricular unloading matter in STEMI-related cardiogenic shock?
New registry data and an accompanying Editorial explore whether "door-to-unload" may emerge as an important metric in shock care.

Intravascular imaging-guided PCI was associated with a significantly lower risk of target vessel failure compared with angiography-guided PCI across all levels of lesion complexity, with increasing prognostic benefit in more complex disease.
Pooled analysis of 4,611 patients from RENOVATE-COMPLEX PCI and registry data.

The SAFE-PROTECT randomized trial demonstrated that the novel AnchorMan device was non-inferior to Watchman 2.5 for 12-month clinical success and left atrial appendage closure, with improved sealing performance reflected by fewer clinically relevant peridevice leaks.

In the PORTICO IDE trial, haemodynamic performance was similar between intra-annular and supra-annular self-expanding TAVI valves.
Compared with the balloon-expandable SAPIEN 3, the intra-annular Portico valve was associated with lower prosthesis-patient mismatch across annular sizes.

A single-session transfemoral TAVI and transfemoral/transseptal TMVR using dedicated devices was successfully performed in a patient with complex multivalvular disease, demonstrating the potential of a fully transcatheter treatment strategy in selected cases.
