Let's talk about "sex" once more. We can begin with basic terminology.

Phenotype by its most strict definition is physical form (observable traits) of an organism emerging from interaction of genotype of an organism (own DNA content) with an environment.

Genotype is a set of all organism's own DNA molecules.

General morphology is geometric conformation/structure of an organism.

Those two things are interconnected. It becomes a bit tricky when we deal with surgeries which change morphology. I would consider them changing phenotype because they change observable traits. However I admit that there is collision of definitions, because surgery doesn't directly change gene expression. However in many cases it changes gene expression because it changes biochemical milieu of an organism (i.e. removal of some organs producing proteins/enzymes which directly or indirectly influence gene expression).

But let's return back to the most strict definition. What can be those environmental factors which interact with genotype? It can be sex hormone forming transcription factor which is a molecular complex enhancing or repressing transcription of a gene to the mRNA (sometimes non-coding RNA) transcript. Gene which isn't transcribed is basically turned off. So transcription factors regulate which genes are switched on and which off.

When it comes to sex related phenotype these transcription factors (TF) are formed by sex hormone receptors activated by bound hormone. Let's take for example androgen receptor (AR). It's a nuclear class receptor protein. When it meets testosterone in cytoplasm of a cell it changes its folded conformation and expose NLS (nuclear localization signal) domain. This is where transport proteins bind and transport receptor-hormone complex into cell nucleus. Here it binds DNA at promoter regions of genes which contain ARE (androgen response element) consensual sequences from outside of DNA double helix and enables assembly of transcription initiation complex which attracts RNA polymerase II which then opens double helix and initiates transcription. And after couple of steps (splicing of pre-mRNA to mRNA) transcript is used for protein synthesis and proteins are building blocks of phenotype, form components of other downstream transcription factors (again can be receptors) etc...

Similar is valid for other sex hormone receptors (ESR1, ESR2, PR...).

So obviously we can see that changing hormonal milieu changes transcription of genes thus it interacts with genotype in formation of phenotype.

Now last thing, what is sex? Sex as I see it is a reproductive strategy of an organism. Thus sex contains all traits responsible for successful reproduction. Most important aspect is of course presence of system responsible for production and transport of gametes (gonads, ductal system, genitals...). But for successful reproduction we need mating strategy which allows utilization of this system. What is important part of a mating strategy?
Secondary sex characteristics such as shape of body, breasts, external genitals, fat distribution, body hair pattern, produced smell, voice etc..
Why? Because without those characteristics phenotypical based attraction will fail and such mammal wouldn't even reproduce. Apparently even sex orientation is based on secondary sex characteristics which are part of phenotypical sex. and this attraction serves purpose of reproduction.

How are secondary sex characteristics induced and maintained? Answer lies in a response to a question how puberty happens. How? Due to hormonal action, due to sex hormones which act as described in above paragraphs. They alter phenotype by altering it's morphological and even other aspects as smell etc. This alteration happens because sex hormones regulate gene expression.
It doesn't matter whether sex hormones are endogenous or exogenous, somatic cells of the body cannot distinguish the source and behave differently based on source itself.
So in the case of exogenously administered sex hormones in case of MtF transsexuals (trans women) they upregulate genes which are typically more active in females and downregulate expression of genes which are typically more active in males. Opposite happens in FtM transsexuals (trans men).

When human changes sex hormonal milieu they change their secondary sex characteristics. In some cases they change them substantially. What does this change means? As I stated sex characteristics are components of sex defined as a reproductive strategy. They are subset of "sex". When we change subset it logically leads to change of superordinate set, in other words alteration of sex characteristics leads to alteration of sex.
And of course it also means that exogenous sex hormones alter phenotype.
