💉PRP vs. Corticosteroid for Shoulder Pain: A Meta-Analysis
An OE Original
Shoulder pain affects more than 1 in 4 people and can significantly impact one's quality of life.
Treatment options include injection therapy. Corticosteroids have been used for decades, however, platelet-rich plasma (PRP) has risen in popularity in recent years.
Which option is better?
We performed a systematic review and meta-analysis of the existing literature on the topic using the OE MIND database!
🏗️METHODS
Our database contained over 764 shoulder-related studies with 122,000 patients!
We found 6 RCTs comparing PRP to corticosteroid injection for pain from rotator cuff disease, tendinopathy, tears, and tendonitis.
All 6 studies used a single injection of PRP or corticosteroid. We performed meta-analyses for composite clinical outcomes (like the ASES and SPADI scores), pain, conversion to surgery, and the incidence of adverse events!
🔎RESULTS
Composite Clinical Outcomes
We pooled the outcomes overall & by time-point. We used an MCID of 6.4 to determine whether the difference was clinically meaningful.
Corticosteroid was better in the first 6 weeks, while PRP was better at 3-6 months!
Overall, the differences between groups were not clinically meaningful, with the exception of early (3-week) scores which were in favour of the corticosteroid group!
Pain
For pain scores, we used an MCID of 14 points on a 100-point scale!
Again, we found corticosteroids to be better in the first 6 weeks, whereas PRP was superior from 6 weeks to 24 months!
None of these differences were clinically meaningful, though!
Conversion to Surgery
No difference in the risk of conversion to surgery was observed!
Adverse Events
No differences in the risk of adverse events were observed between groups!
✅INTERPRETATION
The results of this OE Meta-Analysis found that corticosteroid injection tended to outperform PRP in the short term, but beyond 6 weeks, PRP provided better outcomes!

This overhead view of the USMNT's goal against Iran is sort of a work of art. From the front to back play, to the switched fields, to the well-timed runs, this is some fine 4D chess that GGG's drawn up for his Baby Eagles. (via: @rodrigoccc97)
