This! LMWH often still given at 6pm which means anti-Xa at midnight. Not good for anyone. No need to routinely monitor levels. Only very specific situations e.g. extremes weight, thrombosis despite normal dose, bleeding, renal failure #TeamHaem

Surprised to find many are measuring anti-Xa levels routinely in pts receiving LMWH.
Why do this?
- forLMWHs have predictable pharmacokinetics
- the trials of LMWH did not measure levels so we don’t know the levels to attain
