Scott Galloway is a perfect example of perhaps the most pernicious people in society: the stupid-smart person.

Educated beyond his intelligence. Smart enough vocabulary to justify his ego and smugness, but too stupid to understand basic realities of communists.

Smart people internalize history and make good decisions. They also know the dangers of hubris. Stupid people don't know enough to even opine on the issues in the first place, so their stupidity isn't a threat. But the stupid-smart person can teach at NYU, he can build a big successful business, he can even give a good TED Talk, but he's not smart enough to see the limits of his own capacities...so he asserts himself in society with an outsized level of confidence. Sadly, he's articulate enough to fool lots of people into elevating his stupid ideas...and that's the problem.

People listen to the stupid-smart guys, like him. Then they're genuinely baffled when there's a Kalashnikov pointed at their head as they're staring at a cinder block wall behind the politburo headquarters. In a functional society, this clown would be a pariah. I have precisely zero empathy for his plight in NYC. He endorsed it. Now he can eat it, good and hard.

Long post, but as an ICU RN who has taken place in the process of determining brain death, I am going to explain how it works.

If true what Laura Loomer's source is saying about Mitch McConnell is true, the hospital ethics committee and brain death determining physicians need to step up.

Brain death is determined by at least TWO physicians.

A neurologist and typically a pulmonologist or intensivist, which is an ICU specialist. If neither are available, a Hospitalist can step in as the second.

Series of tests are performed to determine brain, which is legal death.

The patient must be absent of reflexes, where they once had them. If they had prior brain injury and are missing a reflex, that absent reflex doesn't count.

No brain flow (activity). This is determined by the neurologist through advanced imaging.

And an apnea test. This would be where the pulmonologist or intensivist would come in.

In an apnea test, blood gases are drawn and the CO2 must be normal. A normal CO2 is 35-45.

The patient cannot have any prior lung injury. As this could alter results.

The patient is hyper-oxygenated at 100% oxygen for about 10-15 minutes.

The patient is then disconnected from the vent, but still receives oxygen via nasal cannula or through the ET tube. The tube still stays in the mouth, but the vent is disconnected.

This lasts for a set amount of minutes, around 8-10 minutes.

If any distress occurs, such as a change in heart rhythm or blood pressure in which they become unstable, the test is over and they are hooked back up to the vent.

If no distress occurs, the patient remains off the vent for around 8-10 minutes.

They do receive oxygen during this time. They are not only hyper-oxygenated prior, but also receive oxygen during the test. The heart is beating and so oxygenated blood continues to flow to organs.

After the test the vent is then hooked back up and blood gases are immediately drawn again.

If the patient does not breathe on their own during that time and the CO2 rises to 60+ or up by 20, this means they failed the test. They did not make any attempt to exhale and blow off any CO2.

Brain death is then determined, documented, and the time of brain death is documented. Both physicians ensure their documentation is complete.

It is unethical for the attending physicians and the hospital to keep him ventilated if he is brain dead.

If his wife refuses to extubate him, the hospital can do so on their own with the ethics committee or court orders if needed.
