@pimentel_md Urgent cisplatin-based chemotherapy beginning today without orchiectomy. If HCG is massively elevated, many centers would give 3 days of EP and hold for a bit and resume with 5 day course once bleeding risk is past. I council such patients that they may end up in ICU intubated
