Saw some video of a Mumbai beach covered in trash. The trash is coming from the sea during high tide. Not because people are dumping trash on the beach as many think.
This trash is being dumped into the sea by rivers and canals especially during the monsoon. This issue is a world wide issue. Beaches from LA to SG have the same issue during storms and rainy season.
The solution is in installing river and canal runoff nets (storm water netting system) as done by some countries. And ensuring the rivers and canals aren't used as a dump.

The point is, this trashy beaches are a problem world over after storms, rains or during the rainy seasons. Some of the anti-India propaganda machinary is already taking pics from Mumbai and making it "Only in India it is like this. India is a sh1t hole."
My post is not to justify such unclean beaches or the trash. But to let Indians know this is a world wide problem and there are some solutions that are being tried by some countries.

I donated around 60 litres of breast milk to the government hospital in Hyderabad and Chennai during my first year of post partum!!
Why does it matter? Just 100ml of donor milk can feed a tiny 1kg baby for several days. This donation could potentially support dozens of infants in the NICU. Navigating Neurodiversity: Understanding Autism in Early Childhood
As a pediatrician, one of the most common concerns parents bring to my consultation room revolves around their child’s developmental milestones. Over the last 14 years of practice, I've had the privilege of walking alongside countless families navigating the beautiful, yet sometimes overwhelming, journey of early childhood development.
With April marking Autism Awareness Month and today being World Autism Awareness Day it is the perfect time to shed light on Autism Spectrum Disorder (ASD), break down the stigma, and discuss why early recognition is an absolute game-changer for children and their families.
Recognizing the Early Signs
Autism is a spectrum, meaning it presents differently in every single child. There is no "one-size-fits-all" checklist. However, as parents and caregivers, paying attention to early social and communication milestones is crucial.
Here are a few signs that may warrant a developmental screening:
Atypical Eye Contact: Avoiding or struggling to maintain eye contact during interactions.
Delayed Speech or Gestures:Not responding to their name by 12 months, or not pointing at objects of interest by 14 months.
Repetitive Behaviors:Repeating words or phrases (echolalia), flapping hands, rocking, or spinning in circles.
Preference for Solitude: Showing little interest in playing with other children or sharing enjoyment.
Sensory Sensitivities:Having unusually strong reactions to certain sounds, smells, textures, or lights.
The Power of Early Intervention
If you notice any of these signs, the instinct might be to "wait and see." I always advise parents against this.
A child's brain is highly adaptable in the first few years of life a concept known as neuroplasticity. When we identify developmental delays early and introduce targeted therapies (such as speech, occupational, or behavioral therapy), we give that child the best possible tools to build their communication and social skills. A diagnosis is not a label to limit a child; it is a key that unlocks the specific support they need to thrive.
Building a Supportive Ecosystem: our approach always emphasizes treating the whole child, not just a set of symptoms. Supporting a neurodivergent child also means supporting the family. Raising a child with ASD can feel isolating, but you are not alone. Building a strong ecosystem of supportive doctors, therapists, and fellow parents is vital.
Let's use this month to move beyond mere "awareness" and towards true acceptance. Let's create environments where neurodivergent children are celebrated for their unique perspectives and strengths.

In day-to-day practice, conditions like dengue, pneumonia, dehydration, asthma, or infections rarely become severe without warning. The body gives signals early — but patients and families often don’t recognise them.
The gap is not medical care.
It is recognition.
Many families leave the clinic with medicines, but without clarity on what to watch for. When symptoms worsen, they either wait too long or panic too late.
What makes counselling effective is not giving more information —
it is giving the right trigger points.
What works in real practice:
• Highlighting 1–2 red flag signs clearly
• Explaining them in simple, visual terms
• Linking each sign to immediate action
• Avoiding long, confusing lists
• Repeating the most important point before discharge
For example:
“In dengue, come immediately if there is persistent vomiting or unusual sleepiness.”
When families know what to watch for, they act early.
When they act early, outcomes improve.
Good medicine is not only about treatment.
It is about timely recognition.
Clarity prevents delay.
Delay creates complications.

Patients don’t remember guidelines.
They remember what you told them to do at home.
In everyday practice, most clinical outcomes are decided not inside the hospital, but outside it at home.
Fever management.
Dengue warning signs.
Asthma inhaler use.
Dog bite care.
The diagnosis may be correct.
The prescription may be appropriate.
But if the patient or family does not understand what to do next, the outcome becomes unpredictable.
Condition-specific counselling is not extra work.
It is core treatment.
What works in real practice:
• Giving 2–3 clear home instructions, not a long list
• Explaining one key danger sign that needs urgent review
• Demonstrating when technique matters (inhalers, ORS, wound care)
• Using simple, repeatable language
• Setting a clear follow-up timeline
For example:
“In fever, focus on hydration and comfort. Come back immediately if the child is not drinking or becomes unusually drowsy.”
When instructions are simple, families act correctly.
When actions are correct, complications reduce.
Good counselling converts treatment into outcomes.
Medicine begins in the clinic.
Recovery happens at home.
