@AndrewLalusis@KP23114@iamn0tthe1@mahad_minhas Rare that happens but ok. Good for the NP?
Whether or not an IMG took the MCAT, in order to even apply for residency (the SUPERVISED practice of medicine), they still had to pass 2 licensing exams and pass rotations. Not even talking about the bias during the interview process.
@AndrewLalusis@KP23114@iamn0tthe1@mahad_minhas Your example is akin to saying that because the pilot had a rough landing, flight attendants should be pilots because they have some experience with planes.
@AndrewLalusis@KP23114@iamn0tthe1@mahad_minhas Are there still bad apples? Yes, but it’s uncommon. But that doesn’t mean we should allow NPs with less training to take over. The mistakes and bad referrals I’ve caught are at a much higher rate with NPs than MDs, which is in line with published data
@AndrewLalusis@KP23114@iamn0tthe1@mahad_minhas Absolutely not. Low achieving IMGs simply do not match. Bedside experience is not a substitute for medical school as much as NPs want to believe so hard that it is
@AndrewLalusis@KP23114@iamn0tthe1@mahad_minhas Nurses with doctorate degrees can call themselves doctors outside of a clinical setting, even though I disagree with the lack of rigor or standards for their doctorate. Lecturing? Sure. Dinner with friends? Sure. In a hospital with patients? NO.
@AndrewLalusis@KP23114@iamn0tthe1@mahad_minhas The American system allows high achieving IMGs to apply and receive residency slots if they are deemed qualified. The entire formal medical education process (7+ years, full time) teaches the why of medicine, which cannot be said of a 2yr NP program
@KP23114@iamn0tthe1@mahad_minhas Then in my third and fourth year, I saw patients, answered pages, pended orders for my residents to sign, rounded with my team and took call. 80hrs/wk. Study when possible for a national standardized exam every 4-8wks.
@KP23114@iamn0tthe1@mahad_minhas Meanwhile as a medical student, we practiced clinical exam skills on community volunteers with MDs watching us & providing feedback. 8hrs/wk for 2 years. Then we’d practice on our classmates at home. And have clinical exams twice a year.
@AndrewLalusis@KP23114@iamn0tthe1@mahad_minhas Your profession is only as good as the weakest link. For profit direct entry online NP programs are a sham that unfortunately makes all NPs look bad.
@KP23114@iamn0tthe1@mahad_minhas Prove it with unbiased, randomized and well-controlled studies that show that NPs practicing independently leads to better outcomes. There aren’t any. There are many studies showing increased cost of care, more studies and more inappropriate referrals though!
