Former Prime Minister Shri Lal Bahadur Shastri resigned as Railway Minister after two tragic railway accidents. If you don't want to go that far back, here's a more recent example after the 26/11 Mumbai terror attacks, Maharashtra Chief Minister Vilasrao Deshmukh, Maharashtra Home Minister R. R. Patil, and Union Home Minister Shivraj Patil resigned from their posts. They accepted accountability instead of blaming the opposition.
