I agree, most operators have been lulled into the #bigmedicine game and don't even know the options (sometimes better) available in terms of practice model. An ethical and skilled provider can be truly an amazing asset to sick patients in the OBL.

Asymptomatic trial almost impossible in this group, but beg the question, is there a group that would benefit from earlier Intervention?
