A congressman recently described some nonprofit hospitals as “hedge funds with hospital beds.”

My first reaction was: that tracks.

So I started digging.

Nonprofit hospitals don’t have shareholders, but they generate billions in revenue, pay executives millions of dollars, build massive real estate portfolios, and receive substantial tax advantages in exchange for providing community benefits.

At the same time, some nonprofit hospitals (like St. Jude and Shriners) provide extraordinary care supported largely by charitable donations.

So what exactly does “nonprofit” mean in modern American healthcare?

Are these institutions serving the public in exchange for their tax benefits, or has the definition become so broad that it no longer means what most people think it means?
