What a great day of education around quality discharges alongside our community partners! We also launched the MOFD Squad who will be supporting discharges for today and tomorrow, unblocking barriers and improving the patient experience @UHSFT#patientsfirst
Last month, a third of non-clinical discharge cancellations were due to patients having a lack of transport to get home. 🚘❌
This means patients who were medically cleared to leave hospital had to stay longer, simply because they didn’t have anyone to collect them.
