Adult-Onset Still's Disease (AOSD)

This image illustrates the classic triad of Adult-Onset Still's Disease (AOSD), a rare systemic inflammatory disorder characterized by:

🛑High-spiking quotidian fever
The fever is typically the earliest and most prominent feature. It often exceeds 39°C and follows a quotidian pattern, with one or two daily spikes separated by periods during which the temperature returns to normal. Fever frequently precedes the appearance of other symptoms.

🛑Evanescent salmon-pink rash
The characteristic rash is transient, salmon-colored, and macular or maculopapular in appearance. It usually coincides with fever spikes and fades as the temperature normalizes. The rash most commonly affects the trunk and extremities and is generally non-pruritic.

🛑Arthralgia or inflammatory arthritis
Joint symptoms occur in most patients and may initially present as mild arthralgia before progressing to persistent inflammatory arthritis. The knees, wrists, and ankles are most commonly involved, although multiple joints can be affected.

Beyond the classic triad, patients frequently experience myalgia, recurrent sore throat, lymphadenopathy, splenomegaly, hepatomegaly, pleuritis, or pericarditis, reflecting the systemic nature of the disease.

A key laboratory clue is marked hyperferritinemia, with serum ferritin levels often exceeding 1,000 ng/mL and frequently rising to more than five times the upper limit of normal. Elevated ESR, CRP, neutrophilic leukocytosis, thrombocytosis, and abnormal liver enzymes are also common, while rheumatoid factor (RF) and antinuclear antibodies (ANA) are typically absent.

Because no single test is diagnostic, AOSD remains a diagnosis of exclusion. Infectious diseases, malignancies (particularly lymphoma), and other rheumatologic disorders must be carefully ruled out before establishing the diagnosis.

The clinical course may be self-limited, relapsing-remitting, or chronic. Serious complications can occur, including macrophage activation syndrome (MAS), disseminated intravascular coagulation (DIC), pulmonary arterial hypertension, and severe cardiac or hepatic involvement.

Clinical Pearl: The combination of daily high-spiking fever, a transient salmon-pink rash, inflammatory arthritis, and markedly elevated serum ferritin should strongly raise suspicion for Adult-Onset Still's Disease (AOSD).
