Regardless of if Chromosomes are an objective marker of sex it’s not one any society uses to assign sex at birth or to assume one’s gender.

Setting that aside if whatever treatment you’re providing is related to sex characteristics you’d probably just ask if they’re trans or intersex and if so ask what kind or diagnose them. Phenotype tends to matter more than genotype for medical care.
