Fit notes replaced the ‘sick note’ in 2010.

Given they emphasise what is needed for the person to be ‘fit to work’ again, I suggest simply writing:

Universal Credit is coming to Shettleston Jobcentre on 5th December. Due to the built-in 5 week wait, people claiming on day one won’t get any money until at least the 9th of January.

The @DWP should stop the roll out and fix this broken system.
