The classic silver stool (Thomas’ sign) is a rare but memorable clinical sign of carcinoma of the ampulla of Vater. It occurs when the tumor obstructs bile flow, producing pale (clay-colored) stool, while simultaneously causing slow gastrointestinal bleeding. The combination of acholic stool and digested blood (melena) gives the stool its characteristic silvery-gray appearance.

Important points:

1. Silver stool (Thomas’ sign) is a rare but classic sign of ampullary carcinoma.

2. The silver color results from the combination of clay-colored stool caused by obstructive jaundice and melena from chronic tumor-related bleeding.

3. Ampullary tumors obstruct the distal common bile duct, preventing bile from reaching the intestine and causing pale stools, dark urine, and jaundice.

4. Patients typically present with painless progressive jaundice, pruritus, weight loss, and occasionally intermittent gastrointestinal bleeding.

5. Liver function tests usually demonstrate a cholestatic pattern, with elevated bilirubin, alkaline phosphatase (ALP), and often γ-glutamyl transferase (GGT).

6. Diagnosis is established with contrast-enhanced CT, MRCP, or ERCP. ERCP also allows tissue biopsy and biliary decompression when indicated.

Clinical summary:

Silver stool = Pale (acholic) stool + Melena → Think carcinoma of the ampulla of Vater.

You could get a Bildungsgutschein that pays 100% for your B2 Medizin course or FSP preparation.

That’s thousands of euros you don’t have to pull from your pocket or family.

𝘐𝘤𝘩 𝘩𝘢𝘣𝘦 𝘦𝘪𝘯𝘦𝘯 𝘛𝘦𝘳𝘮𝘪𝘯. = I have an appointment. 𝘐𝘤𝘩 𝘣𝘳𝘢𝘶𝘤𝘩𝘦 𝘦𝘪𝘯𝘦 𝘉𝘦𝘴𝘤𝘩𝘦𝘪𝘯𝘪𝘨𝘶𝘯𝘨. = I need a certificate. 𝘌𝘯𝘵𝘴𝘤𝘩𝘶𝘭𝘥𝘪𝘨𝘶𝘯𝘨, 𝘪𝘤𝘩 𝘷𝘦𝘳𝘴𝘵𝘦𝘩𝘦 𝘥𝘢𝘴 𝘯𝘪𝘤𝘩𝘵. = Sorry, I do not understand.

German offices run on paper and patience. Bring every document, bring copies and arrive early.
