I have just finished another set of incredibly difficult weekend nights as the A&E consultant incharge.

Some thoughts:

The NHS no longer provides unscheduled care - we provide treatment. Care is with dignity - that's impossible for our patients in corridors.

The model of acute care is not working or sustainable. We need a completely different approach - with long term investment in prevention and community care

111 need to change risk thresholds as far too many come to a&e when they don't need to.

There needs to be alternative pathways to get care overnight for those who don't have accidents or emergencies. A&E shouldn't be anything and everything

Staff are broke - supporting staff is just as vital as looking after patients

This government have totally decimated our the NHS. They are the ones who should be apologising to our patients not our staff.

Hundreds are dying each week because of lack of hospital capacity and corridor care in a&e, yet it's not on the news and no public uproar. As staff and patients we have accepted the unacceptable.

Everyone needs to know the reality of what is happening - so that politicians offer a different solution and patients get the care they deserve. Sadly most of the public have no idea of what is genuinely going on.

And finally, a post night fry up is the best antidote for the stresses of the weekend. Goodnight!

The budget is just 6 weeks away. Today I sent the Chancellor, @Jeremy_Hunt a letter, on his request, to ask that he fixes:

- Unfair Child Benefit rules penalising single income families
- The Lifetime ISA penalty hitting young 1st time buyers
- Above inflation mid-contract broadband & mobile price hikes
- The substantial real terms cuts to student living loans.

Please read the letter for full info. I hope he listens and we see change in the budget. He can't now say he wasn't told. I will also be sending the letter to @RachelReevesMP

Dear @elonmusk not sure you're aware that reader's notes are being put on scams. I know you're used in scam ads just like me. Can I ask, do you approve of this method of policing?

Can X please delete and ban scammers. I'm sure you don't want their dirty money for this paid promotions.
Please can you look at your policy on this, to help protect vulnerable users.

I used to write as a compulsion. But in recent history, I've fallen down the hole of promising myself that I'll be a writer when I free up the necessary time for such a commitment. But in so doing, I have stopped writing. I have lost the habit. I need to make time to re-engage.

@RishiSunak This speech seems to be patting yourself on the back for policies that have yet to make any significant difference. One can only wonder who you were addressing (in reality) when suggesting you are to put 'their' priorities first. A little transparency on who really matters to you

Since Boxing Day I have read two books: No Plan B and A Heart full of Headstones. The latter I have just finished. An excellent read. Still a few of the back-catalogue to read. @Beathhigh @LeeChildReacher
