When your PBM says it "passed through 100% of rebates," the load-bearing word is "received," not "paid." A rebate aggregator it owns takes a cut first. 100% of what's left is still 100%. The math is honest. The noun is not.
CVS Caremark dropped Zepbound for Wegovy as its preferred GLP-1.
The head-to-head trial favored the drug they dropped.
So this wasn't a clinical call. It was a rebate deal, and members lost access to the more effective drug so the PBM could collect on the contract. "Preferred" doesn't mean better. It means it paid the most to wear the label.
Do you know your plan's generic spread? Not the discount. The spread is the gap between what your PBM pays the pharmacy for a generic and what it bills you. Most plan sponsors have never been shown it, because it is rarely a line on the report.
