No one is convulsing. The explanation is actually simple. Touching the Black Stone has no magical impact on a person’s life. Muslims don’t believe the stone brings benefit or harm. It is only done as a symbolic act of following the practice of the Prophet Muhammad SAW during the counter-clockwise direction walking around the Ka'bah.

That’s why Umar ibn al-Khattab said:
“I know you are a stone that neither harms nor benefits. Had I not seen the Prophet SAW kiss you, I would not have kissed you.”

So the “impact” is spiritual obedience, not superstition. And if the crowd is heavy, Muslims are taught to simply point toward it and move on.
