Saturday, Yesterday.
Saturdays are my getaway days from the routine harsh realities of life and death that happens in my daily practice from Monday to Friday. Saturdays, we - me, wife and kids, we try a new restaurant or let the kids play at the mall.
But this Saturday, I had to be in my outpatient services because I was on leave for some personal matters on Tuesday.
A Saturday I will not forget.
A patient of mine, a 49 year old man advised for liver transplantation since three months and on best supportive care since then brought his wife, aged 41 years, to the emergency department.
They have two children, a girl and a boy, aged 11 and 8. The family was just making ends meet and so he made it clear that he could not afford the 2,000,000 INR liver transplant and he was ready to face whatever was in store for him.
During the last 18 months, since the diagnosis of his advanced liver disease and last six months of liver failure, his wife has been his constant support. Everytime he came in for a review, she was there. She knew his reports from six months before by-heart. She tabulated his reports and trends on a large notebook and listed his new symptoms and clinical issues. She knew he was dying without a life saving transplant, but she would always talk cheerfully and point out very minute, irrelevant improvements from the blood tests in front of him to keep his spirits high.
But what we did not know was that she was suffering from recurrent pain abdomen, lethargy and overall ill-health since many months, but did not care to get hereself checked up because it would be too much for the family to handle - financially and mentally.
Saturday, yesterday, in the emergency department, she was brought in with severe jaundice, high spiking fever and breathlessness. She crashed in front of the emergency physicians who tubed her and put her on the ventilator. An infection was ripping through her and the source of it was her liver. She had a condition called primary sclerosing cholangitis which was being treated with some symptom care nearby, and for which further investigations were not performed because she wanted to save money to get her husband's life quality better, knowing well, he was dying without a liver transplant. No one knew about this diagnosis, until we made it in the medical ICU on an already dying woman.
Her husband, my patient was dumstruck at the pace at which the infection took her into a state of multiple organ failure.
While he sat in front of me on this Saturday, yesterday afternoon, he did not say anything. I tried pacifying him, but I knew no words would be strong enough. His children were on their way, to see their mother on the ventilator. It was their last chance to say goodbye before her heart gave out. She was going to die. I asked him what he was planning to do and say. He kept quite.
I asked him if he has a sister or a brother. And he said he had both. I tell him that in a matter of 3 to 6 months he would die too, because his liver failure was advanced and a timely transplant could only save him. I told him, it was time to think about the children, because he would have to be in hospital for his complications in the coming months. He told me, he was happy that I was talking practical and thinking about his family, for him, at a time when he could not gather any thoughts inside his head.
He said he knew that his children had to be priority now. So Saturday, yesterday, I asked my slowly dying patient, to make ready, his will, for custody of his children with his elder brother or younger sister, because his wife who took care of his illness, by lovingly neglecting hers, was going to die soon.
The wife, their mother died last night due to a condition called cholangitis, a severe form of infection that affects the bile system in patients with primary sclerosing cholangitis, a dreaded disease of the liver. No antibiotic could save her. The bacteria ripped her apart without giving us any time to salvage her. She came in so sick and within 12 hours she was dead.
And he will be too. And then the children will be without both their parents before this year ends.
I used to love all my Saturdays, just before yesterday's because it was a special time with my wife and children, away from the hospital chaos. It was a time when we were a family, and nothing else mattered and we had our own frame to be in. But now I fear I will keep wondering in melancholy, about the children who will be without their parents, like my patient's in the near future, on every Saturday, like yesterday.
As I move into the experience of both caring and learning that clinical medical practice gives me, the same practice seems to be chipping away at every little thing that I hold on to, as a sanctuary.
