“This is similar to what is being seen in many UEC settings where some areas are being rebranded to "off the clock" areas improving performance data but not changing the patient journey or experience.
@acutemedicine people. We’re hearing of increases in AMU bypass due to boarding. So patients who are frail and vulnerable are day 8 on AMU whilst those we can discharge within 72 hours are going direct to specialty. This gamification is really concerning.
I'm keen to know if I’m just stating something that’s obvious here, or whether I’ve stumbled across a correlation that might be useful in our efforts to understand patient flow…

Twenty years ago we had a geriatric day hospital where we could see patients and give transfusions. Managers closed it. Now the new thing is SDEC where we can now give transfusions and see patients. Managers love it.
There are now more female than male doctors in the UK, a significant milestone for the profession and for patients.

Snowy Durdle door in Dorset
One of my most memorable moments, capturing this very rare image. The fact that we never really get snow down south and especially at such an iconic tourist attraction such as Durdle Door!

@RCollEM@acutemedicine Elderly/frail patients should not be excluded from SDEC. Can interfacing with frailty teams be an idea? Rather than lying around in the corridor of the ED?
Being an inpatient carries real harms, often avoidable.
Some might remember my proposed education initiative to improve feedback and the general education experience for doctors in acute medicine last year.
Well, I have now been running the ‘feedback clinic’ for a year, and I wanted to share my learning/experiences
Oh wait through…. It’s a Johnson piece, facts and truth don’t count right?
And that’s not even mentioning the flexibility WFH affords …. to most often women and those with caring responsibilities… but let’s not let a little misogyny stand in the way of clicks.
I cannot do an eye roll big enough.
She met him at a Christmas party at her parent’s house where they had been neighbours for years.
She was exploited and sexually assaulted by her boss at work.
Have you not even heard of supermarket dating? 🍍
WFH may be OK for the older generation, but for the Bridget Joneses of today it's a sham and a snare - and they'll never meet their Mr Darcy
