The Tampa Bay Rays are 26-13, the best record in the AL.
Their payroll ranks bottom 3 in MLB at $87 million. Their highest-paid player is Yandy Díaz ($12M).
Every year they find a way to compete with teams spending 3x as much. One of the most impressive organizations in sports.
When you have an ESBL that necessitates Zerbaxa use (e.g., mixed ESBL-E and DTR-P. aeruginosa infection), do not assume ESBL susceptibility! Especially if K. pneumo present

“Goal trough is typically 15–20 mg/L”?
Surprising that made it past peer review — not just outdated, but potentially harmful!
Studies consistently show that chasing those high troughs increases nephrotoxicity without improving outcomes once AUC is optimized @BradSpellberg
@BradSpellberg@DrToddLee@DiorIzzy@ABsteward@CMIJournal It’s always challenging when you get the cognitively impaired pt who’s mention worsens and staff or family reporting “they always get like this when they have UTIs” and trying to convince the provider not to treat.
@psirides Great chart! Might I suggest editing the ESCHAPPM section regarding AmpC producers. IDSA recently provided guidance on treating AmpC producers and suggests abandoning acronyms, citing that only E. cloacae, K. aerogenes, C. freundii should she be considered moderate risk for AmpC.

Received this from IDSA. Please consider sending your concerns to congress.
ACIP appears to be dismantled because of politics. The notion that widespread conflict of interest is present on FDA committees is baseless (as a former committee member).

Kucherov on his teammates wanting to train with him in the offseason: "If you want to come, it's next week."
followed by laugher from the crowd and another question of what that means to him
"Again, it starts next week. If you come in August, it is like, 'Well, I have been working for three months.' You know? It is good to have guys come back earlier but, at the same time, I am not the coach. It is really hard to, I got to work on my stuff but, at the same time, you have to help the other guys. It is tough but I am happy guys are willing to come earlier and put work in because we have free ice, free gym, everything you want here. The trainers are here. I think it is the best place to put everything outside and just focus on your game, your skill, and what you got to be better at. It is good to have young guys come. Hags came over last summer and he had a hell of a season ... we spent a lot of time on the ice together and we did so many reps together. I think it helped him feel more confident on certain plays and he is a hell of a player and he proved it this year and I think he still has a lot of room to improve and be a way better player than he was this year"

@IDPharmD_Kate crushing it & getting everyone talking about, is this really AKI when it comes to vanco + pip/tazo? And coining the term fake-KI 🤩 Her key lesson…we probably don’t need vanco (or pip/tazo) for most patients! #IDWeek2024#SIDP2024
