La nefroprotección moderna ya no es solo “controlar creatinina”.

En ERC, debemos pensar en capas terapéuticas:

1️⃣ IECA/ARA-II si albuminuria
2️⃣ iSGLT2 si eGFR lo permite
3️⃣ Control intensivo de PA
4️⃣ Finerenona en pacientes seleccionados
5️⃣ Monitoreo estrecho de K y eGFR

El objetivo: frenar progresión, no solo observarla.

¿Cómo llevar finerenona a la consulta nefrológica?

Paciente ideal:
ERC + albuminuria persistente + eGFR ≥25 + K normal + IECA/ARA-II optimizado.

Dosis práctica:
10 mg/día si eGFR 25–<60
20 mg/día si eGFR ≥60
Control de K y eGFR en 4 semanas.
