🌐En el marco del #MesDelCancerSanguineo, mañana sábado, 5 de septiembre, conmemoramos el Día Mundial del Mieloma Múltiple.

Hoy contamos con nuevas estrategias terapéuticas que permiten controlar la enfermedad, alcanzar respuestas cada vez más profundas y prolongar la supervivencia, acompañada de una mejor calidad de vida.

🔬 La investigación ha sido clave en esta revolución. Y la investigación española ha realizado contribuciones decisivas que han ayudado a cambiar la práctica clínica y el manejo de esta enfermedad.

Landmark trials deserve landmark scrutiny.

The PERSEUS trial has transformed frontline treatment for transplant-eligible multiple myeloma and established a new standard of care.

But practice-changing does not mean scientific discussion should stop.

As clinicians and educators, we should distinguish observed evidence from model-based projections, MRD negativity from cure, and trial populations from real-world patients.

The goal is not to criticize a landmark trial.

The goal is to better understand it.

For years, we dreamed of off-the-shelf CAR-T.

Then in vivo CAR-T walked in carrying a nano-parcel. 🧬📦

2 studies. 2 diseases.

KLN-1010 in multiple myeloma:
ORR 100%, 14/18 MRD-negative

LB2501 in lymphoma:
ORR 100%, CR 83%

No apheresis.
No manufacturing.
Rapid access.
Potential outpatient delivery.

Too early to declare victory.
Durability and late toxicity are still the dragons.

But the factory may be moving into the patient.

My hypothesis: IEC still matters.
The ecosystem may decide how long the army stays.

Are we witnessing the beginning of the end of autologous transplant… or simply the beginning of a more selective transplant era?

Anti-BCMA T-cell engagers are transforming newly diagnosed multiple myeloma. An exciting new era indeed.

The published MajesTEC-5 study is impressive, but it remains a 49-patient, non-randomized phase II trial. It lacks patient characteristics directly comparable to contemporary transplant studies and, more importantly, has no mature data on 2 to 3-year sustained MRD negativity, durability, or overall survival. For me, this is not yet practice changing.

Interestingly, the randomized phase III MajesTEC-4 study is asking a different question. Its safety run-in has also reported nearly 100% CR and 100% MRD negativity following ASCT. These two approaches are now competing to define the future standard.

My prediction? Autologous transplant will become more selective, not disappear.

I will continue to advocate stem cell collection for eligible patients. Cryopreserved stem cells may still prove valuable for salvage ASCT in selected patients, stem cell rescue for prolonged cytopenias after CAR-T or other cellular therapies, and future indications we have not yet anticipated.

The future belongs to immune redirection. But durable remission, sustained MRD negativity, and overall survival, not early response rates alone, will ultimately determine whether transplant becomes optional.
