In a cohort of >500 patients with persistent FN, new BSI after FN day 3 was identified in 4% of patients and 2% of days with cultures collected. FN onset in the peri-HCT period (day -7 to +30) was associated with 80% lower odds of new BSI after FN day 3. Among new organisms in blood cultures after FN day 3, 26% were contaminants with over half leading to excess vancomycin use, highlighting the importance of diagnostic stewardship in this population.
