Are we measuring surgical safety checklist use accurately? A recent study compared EMR documentation with OR Black Box® video observations in gynecological surgery and found a notable gap between what is reported and what actually happens in the OR.
While documentation suggested high compliance, video analysis showed:
• Lower overall quality of checklist execution
• Variable team engagement
• Only 80% completion of the Sign-out phase
Most striking, checklist quality was recorded at 89% in the EMR versus 47% when assessed through direct observation.
These findings raise an important question. Do we need a more objective “co-pilot” to ensure safety processes are not only documented, but meaningfully performed?
Bridging the gap between perception and reality is key to advancing patient safety.

Using OR Black Box® data from over 4,500 procedures, a research study published in Surgical Endoscopy found that higher compliance with the surgical safety checklist is associated with:
• Lower mortality
• Shorter hospital stays
• Reduced ICU admissions
Importantly, specific phases of the checklist matter. Strong performance during timeouts and debriefings showed independent associations with improved patient outcomes.

At the ASMBS Annual Meeting, SST co-founder Dr. Teodor Grantcharov spoke in a session titled “No Longer The Future: AI, Digital Surgery, Machine Learning and Quantum Computing in Today’s OR.”
Rather than exploring what’s next, the session emphasized what is already here:

Onsite at @StanfordHealth , we partnered with perioperative leaders for a workshop focused on how AI-driven, ambient technology can drive OR management within existing workflows - not by adding complexity, but by working within it.
The session was anchored in real use cases and a hands-on case study built around a realistic end-of-day OR overrun. Using the SST platform, participants ran an interactive investigation - exploring scheduling accuracy, tracing how delays unfolded across the day, and conducting structured root cause analysis. They then translated those insights into practical ideas for operational improvement.
The takeaway? When OR teams have access to continuous, trustworthy, AI-driven insight, they gain a fundamentally stronger way to manage complexity.
