The first known gender-affirming surgery, an orchiectomy (removal of the testicles) as part of a male-to-female transition, was performed on Dora Richter in 1922 in Berlin, Germany.

The first complete sex reassignment surgery, including penectomy and vaginoplasty, was also performed on Richter in Berlin in 1931.

These procedures took place under the auspices of the Institute for Sexual Research in Berlin, founded by Magnus Hirschfeld, which is recognized as the world's first trans clinic and the site of the earliest modern gender-affirmation surgeries starting around 1930.

Similar early surgeries, such as those on Lili Elbe beginning in 1930, were also conducted in Germany, with her initial procedure in Berlin and subsequent ones in Dresden.
