#Weekend_review#menin_inhibitor#leusm 🧵 Menin inhibitors in AML: biology → approvals → combinations → resistance
1/9
Menin inhibition has rapidly moved from a biological concept to an established therapeutic strategy in AML.
Why does it work?
KMT2A-r and NPM1-mut AML are genetically distinct—but both converge on the HOXA/MEIS1 transcriptional program, which maintains leukemic stemness and blocks differentiation.
Menin helps sustain this program.
Menin interacts with KMT2A and helps maintain transcription of genes including HOXA9 and MEIS1.
Blocking the menin–KMT2A interaction suppresses this leukemic program and promotes myeloid differentiation.
Importantly, mutant NPM1 also creates a transcriptional state dependent on this axis:
KMT2A-r → MENIN/HOX-MEIS ← NPM1-mut
Think of menin inhibition fundamentally as a differentiation strategy, not simply cytotoxic therapy.

🧬 Post-alloHCT maintenance in AML @Mark_J_levis from @HopkinsMedicine
🔹 Strongest evidence in FLT3-ITD AML
🔹 Benefit driven by MRD status + immune synergy (IL-15, GvL)
🔹 Not all targets are equal: FLT3 > Menin (emerging) > IDH (selective)
🔹 Parenteral AZA → negative randomizsed data
🔹 Oral AZA (CC-486) → no proven OS/RFS benefit post-HCT
Take-home:
👉 MRD-guided, biology-directed maintenance should be the goal.

What a great way to wrap up the congress of the French Society of Hematology #SFH2025! Gaining insights from Michel Sadelain on the past and future advancements of CAR-T cells.
Graft-versus-host disease can occur post stem cell transplant, affecting quality of life. Low awareness of the diverse #GVHD symptoms may potentially impact diagnosis and outcomes.
On this #GVHDDay, we recognize the importance of raising awareness to prompt patients to seek help
#GVHDDay makes this the ideal time to learn the ABCs of Graft-Versus-Host-Disease.
Sometimes severe, this complication is experienced by about half those transplanted with donor cells.
