The question arose on this platform (@iceman_ex and @Acute_Pulmo_Med): what is the effect of the use of LUS on occurence of ARDS? Here is the answer: https://t.co/oHOFhTBc7j
⏱️Conventional CPR in cardiac arrest only generates a fraction of CO... this is why it is called low flow time. Longer cCPR associated with worse outcome, also if for pts undergoing #ECPR: keeping <20' better, after 45' survival goes down, but we can push to up 60'. When switching from cCPT to ECPR, meaning when to start cannulating (NOT to start moving team)? How to decrease low flow time being on pump quickly?
🚑bringing #ECMO to patient? prehospital cannulation associated with significant resource use but avoid transport of pt with ongoing CPR
🚑bringing patient to nearest hospital meeting #ECMO team there?
🚑bringing patient to #ECMO center?
🏥cannulating in ED or cath lab?
Whatever environment you choose to cannulate, success of the program mostly depend on organized/trained EMS system, rapid identification of eligible patients, early activation of experienced #ECLS team, comfortable with cannulation location, no matter which this is!

#ECMO & #ECC02r domain, led by @dfmcauley, 2 PICO questions:
🩸in adults with severe #ARDS or #COVID19, does VV #ECLS vs conventional protective MV reduce mortality? change! strong recommendation, moderate level of evidence in favour
🩸in adults with severe ARDS, does ECC02r vs conventional protective MV reduce mortality? new recommendation against use outside RCTs (due to increased adverse events for ECCO2r in REST trial)
