One of the hardest parts of hospital stays is the loneliness.
Not because you’re physically alone. Hospitals are full of people. Nurses, doctors, porters, cleaners, other families. There is always noise somewhere. Always movement.
But when it’s late, and the lights are dimmed,

A 53-year-old woman presented with pain, swelling, and discoloration of her finger. Physical examination showed a demarcated, brightly erythematous plaque. What is the most likely diagnosis?
Papuloerythroderma of Ofuji (PEO)
✅Key Clinical Features
Widespread, intensely pruritic (itchy) eruption consisting of flat-topped, polygonal, erythematous to brown papules that coalesce into large confluent sheets or plaques
Characteristic sparing of skin folds (e.g., axillae, groin, antecubital fossae) and often periumbilical areas, creating the classic "deck-chair sign" — bands of uninvolved skin resembling slats on a deck chair.
The eruption often covers most of the body surface but typically spares the face, palms, soles, and major skin creases
Common associated findings include peripheral eosinophilia, elevated IgE levels, and sometimes lymphadenopathy or scalp hyperkeratosis.
The exact cause is unknown, but it is considered idiopathic in many cases. It can be primary (no underlying trigger) or secondary.
🔸Association with Lymphoma
Yes, Papuloerythroderma of Ofuji is frequently associated with underlying malignancies, particularly lymphomas
A strong link exists with cutaneous T-cell lymphoma (CTCL), such as mycosis fungoides or other peripheral T-cell lymphomas. In some instances, PEO is thought to represent a paraneoplastic syndrome or an early/pre-lymphomatous manifestation of CTCL
Cases have been reported where PEO evolves into or coexists with CTCL, Hodgkin's lymphoma, chronic lymphocytic leukemia, or other lymphoproliferative disorders
Approximately 40% or more of PEO cases may have an associated underlying condition, including lymphomas (one of the most common malignancies linked), solid organ cancers (e.g., gastric adenocarcinoma being frequently noted), infections (e.g., HIV, hepatitis C), atopy, or drug reactions

A young Dutch medical student swam in the Nile River.
Two months after his return, he presented with a nonproductive cough and malaise, as well as diarrhea and nonpruritic rash.
Thoughts?
