PXA! Dural tail/reaction🐒 is the big clue for the diagnosis over other cortical based lesions such as gangliglioma and DNET. These are WHO Grade II tumors that most commonly present as seizures in young people. Remember they can disseminate in CSF!🧠

A case of OPLL! When you see a case of cervical spinal stenosis on MRI, make sure to check if there is T1/T2 dark vertical tissue in the anterior epidural space that would suggest the diagnosis. CT can be done to confirm but probably not necessary in such obvious cases!
Just another day of work during the pandemic. Covid just dominates my practice now. I can hardly imagine how horrible things must be for my ID, ICU and ED friends and their patients.
DDx of adult 4th ventricular tumor include mets, CPP, ependymoma, subependymoma. This one has a good look for ependymoma as it's enhancing + squeezes through the lateral recess. Intratumoral hemorrhage + IVH are rare but known complications, with acute neurologic deterioration🧠
Answer: Ependymoma with intratumoral and intraventricular hemorrhage! Note the lower SWI image that showed layering hemorrhage in occipital horns. Ependymoma is most commonly diagnosed in children (esp posterior fossa subtype) but can certainly occur at any age.
