He did not get into MBBS.

Atharva Karulkar had wanted to work with patients since he was young. When the medical seat did not come, he took a BSc instead, drifted towards immunology, and joined IIT Bombay as an MSc student without a clear goal in mind.

Then he needed a dissertation project.

"I somehow ended up in Rahul sir's lab to find a research project," he says. It was there that he first heard of something called CAR-T therapy.

CAR-T is often called a living drug. A patient's own immune cells are drawn out, genetically reprogrammed in a lab to recognise cancer, and infused back into the body. For people with blood cancers that have returned after every standard treatment, it can be the last door still open.

For most of the world, it was also unaffordable. In the United States, a single treatment costs about $400,000.

Prof. Rahul Purwar had watched the field take shape during his postdoctoral years at Harvard. Back at IIT Bombay, he did something counterintuitive. Before building the therapy, he started work on the lentiviral vector, the engineered virus that carries new instructions into the cell, because buying vectors from outside manufacturers was one of the things driving CAR-T's cost so high.

The research began in 2013. Karulkar stayed on for his PhD. In 2018 they spun out ImmunoACT and ran clinical trials with Tata Memorial Centre in Mumbai.

On 13 October 2023, India's drug regulator approved NexCAR19, the country's first CAR-T therapy. Around 70% of trial patients responded, with no neurotoxicity, a meaningful improvement over approved global therapies of the same class.

It launched at ₹42 lakh and now costs around ₹30 lakh. By mid-2025, more than 350 patients had received it across 70 hospitals, and ImmunoACT turned a profit in its first full year of operations, something almost no Indian biotech manages.

"This is like a dream come true," Purwar said when approval came.

He never got to wear the white coat.

But he helped build what doctors now reach for when every other treatment has failed.

Bihar Minister and BJP MLA from Jamui Shreyasi Singh implied that what happened in Jamui was a form of moral policing, but argued that the way it was done was absolutely wrong and should have been carried out in a "more decent and elegant fashion."

What a messed-up thing to say! Firstly, moral policing is not acceptable, no matter how "elegant" it sounds. Secondly, what happened in Jamui was not moral policing, it was molestation and attempted rape. Anyone bringing "moral policing" into this debate is only diluting the crime.
