⚡️ Glucocorticoid-Induced Osteoporosis (GIOP): 2025 Update ⚡️
•💊 Most common drug-induced osteoporosis → affects 30–50% of chronic GC users.
•⏱️ Fracture risk rises in 3–6 months of therapy; risk not fully reversible post-discontinuation.
•⚠️ Even low-dose pred (≥2.5 mg/day) increases vertebral fractures; ≥7.5 mg/day = 5x vertebral, 2.2x hip fracture risk.
•🦴 GIOP fractures often at higher BMD than PMO �� microarchitecture, marrow adiposity & vascular damage play central role.
•🧮 Assessment: FRAX + DXA = baseline; adjust with TBS, FRAXplus; vertebral imaging essential.
•🧪 Labs: Vit-D, calcium, renal, thyroid, CRP/ESR; bone turnover markers ↓ formation (OC, P1NP).
•💪 Prevention: lowest GC dose, taper early, exercise (spine BMD benefit), Vit-D + Ca supplementation, lifestyle optimisation.
•💊 Treatment landscape:
•Bisphosphonates (alendronate, risedronate, zoledronate) = first-line, proven fracture reduction.
•Denosumab → greater spine/hip BMD gains vs risedronate.
•Teriparatide → only anabolic with proven vertebral fracture ↓ (0.6% vs 6.1%).
•Romosozumab / Abaloparatide → emerging anabolic strategies.
•🧭 Guidelines (2022–24): Early therapy at GC start; anabolic-first (Teriparatide, Romosozumab, Abaloparatide) in very high-risk:
•Prior fractures, BMD T ≤ –3.5, FRAX MOF ≥30%, Hip ≥4.5%, or high-dose GC ≥30 mg/day.
•🔮 Future directions: Dual anti-sclerostin/DKK1 antibody (AGA2118), CCN3 neuropeptide (boosts osteoblasts), selective GR agonists (SEGRAs).

The association between #hydroxychloroquine (HCQ) and major major congenital #malformations was examined in systemic #lupus erythematosus (SLE) and #rheumatoid #arthritis (RA) pregnancies in Sweden; this population-based cohort study included 1007 births (453 exposed) and 2500 births (144 exposed) in the SLE and RA cohorts, respectively. First-trimester HCQ exposure was not significantly associated with malformations, with no clear malformation patterns observed. Results remained similar when considering HCQ use before pregnancy and/or varying malformation assessment periods. These findings confirm that HCQ's benefits may outweigh the risks in managing SLE or RA during #pregnancy.
