Birmingham mother Laura Heath, 40, jailed for 20 years for gross negligence manslaughter of her asthmatic 7-year-old son Hakeem Hussain in November 2017. Heath used her son’s asthma inhaler to smoke crack cocaine and ignored repeated pleas from medics to manage his condition.

Vue Cinemas fined £750,000 over the death of 24yo Ateeq Rafiq at Star City in Birmingham in March 2018. He was trapped under the footrest of an electric seat which had its closing mechanism incorrectly fitted. He’d put his head under to retrieve his mobile phone and keys.
