Delay in patients' blood tests.
Antibiotics not administered on time.
"Inadequate" patient resuscitation when someone collapses.
These problems at HOSHAS in Temerloh, Pahang, compromising patient safety, are due to a "serious crisis" of housemen and medical officer shortages.
Doctors say the general medicine department will have "zero" house officers by the end of August, amid an ongoing transfer of housemen from HOSHAS, the second biggest hospital in Pahang, to state hospital HTAA in Kuantan and the suspension of new HO placements for HOSHAS this year.
"Patient care is already compromised and has become unsafe," a HOSHAS specialist named Dr Rey (pseudonym) told CodeBlue.
Dr Rey warned MOH that HOSHAS is heading towards a potential "medico-legal minefield".
In a Feb 16, 2024 letter to the Pahang state health department (JKNP), HOSHAS general medicine department head Dr See Chee Keong said the halt in new housemen placements has "directly affected" the department's services. At least 34 HOs are needed; the medical department had only 20 then.
"Current medical and house officers are facing workloads that affect quality of service, as well as their motivation, and mental and emotional health," he wrote.
"Medical officers are forced to take over duties that were previously undertaken by house officers, which further adds to their workload until some officers are forced to work overtime. A few medical officers are facing burnout, while others are contemplating resigning from the medical service due to high workloads and an unconducive working environment."
In another letter, on July 10, 2024, Dr See pointed out that HOSHAS' general medicine department only had 20 MOs, amid its "dire" situation in maintaining continuity of inpatient treatment. In his earlier February letter, he said the department had 24 MOs then; at least 46 MOs are needed.
This means the medical department currently has less than half (43%) of its required MO manpower and needs another 26 MOs at least, barring any further resignations.
"This situation has caused serious problems within the department; all the medical officers are under significant stress due to a multiplied workload without housemen."
Dr Benny (pseudonym), an MO at HOSHAS, said the shortage of HOs across the government hospital has led to each medical officer handling “nearly three times” their usual workload. MOs across departments are even put on-call, just for blood draws and procedures.
"Medical officers are working around the clock, often well beyond regular hours, to complete daytime tasks," Dr Benny told CodeBlue.
"This has compromised patient safety and care quality, with regular blood tests reduced and increasing patient complaints about service quality on SISPAA."
Jerantut and Jengka Hospitals in the Central Pahang hospital cluster are set to deploy MOs to cluster lead hospital HOSHAS in a mobilisation plan scheduled to take effect on August 19.
The mobilisation plan involves sending one MO from each Jerantut and Jengka Hospitals to HOSHAS for 15 to 30 days, where they will assist with patient care and on-call duties.
But Dr Max (pseudonym), an MO at Jengka Hospital, said this will worsen staff shortages at their own hospital instead, especially since an MO from the general medicine unit, who has quit, will be leaving in early Sept. Jengka Hospital's medical unit should ideally have six MOs; it will be left with four after the resignation of one MO and if another is sent to assist HOSHAS.
"With two wards housing at least 30 to 40 patients each and additional duties like clinic work and patient referrals, losing a medical officer will compromise patient care. This will lead to longer wait times, less time per patient, increased risk of errors, higher complication rates, and more readmissions, which is already happening," Dr Max told CodeBlue.
HOSHAS specialist Dr Rey said some MOs were so burned out on one particular morning that they couldn’t even take morning bloods from patients.
"We are reaching our breaking point."
