While engine failure played a role as to why the plane crashed, the crash was an avoidable one. The decision of the captain to continue with the flight despite the fact that an engine had failed 15 minutes after takeoff, coupled with poor situational awareness and crew resource management led to the crash.

The captain himself had a poor history. He used to fly for Spirit Airlines some years prior to joining Dana. He was involved in one incident that led to the FAA suspending his license. The incident happened when he hard landed an A320, the landing was so hard that the oxygen masks deployed but this was concealed. Airbus detected an unusual stress data from the aircraft and reported same to the airline. This led to an investigation, consequently leading to the suspension of his license by the FAA.

This was concealed from Dana and Dana was criticized for poor recruitment oversight and vetting procedures.
