🤖 AI can now analyse thousands of patient safety reports in seconds and it's surprisingly accurate.
A new study by Chen and colleagues shows GPT-4o achieved an average agreement of 94% between reviewers in identifying problems from 9,357 free-text incident reports. The model:
✅ Extracted problems from complex narratives
✅ Created a taxonomy of safety issues
✅ Revealed previously hidden patterns
✅ Identified system-level risks
The AI-generated insights were described "clear, appropriate and valuable" with few perceived barriers to adoption.
The game-changer? Almost all incident reports (99.9%) are classified as low-harm events. Although these are rarely examined in depth, LLMs offer a way to unlock their latent insights, enabling proactive, data-driven quality improvement, accelerating manual report triage, and supporting system-level learning from events that are currently overlooked.

Mental illness was not invented by drug companies. People have described and struggled with these conditions long before modern medicine, across cultures and centuries. Reducing them to “bad choices” or “bad people” ignores genetics, early life experiences, trauma, brain differences, and social context.
It’s fair to question overdiagnosis, labels being stretched too far, or medication being used when it shouldn’t be. Those are real concerns but calling mental illness itself a fraud dismisses the genuine suffering and loss of functioning for many people who live with doing everything they can to cope and live well.
