This happens with a lot of people. I've been guilty of things.
I've tried various things to overcome this... but none worked better than the following.
Once my ideation phase is complete, I put up a kanban board... with all the things needed to "complete" the project.
just delete your CLAUDE․md every 3 months and rebuild it from scratch
the model under it changes almost monthly... it has new training, new thinking
and the rules you wrote to correct the old one are most likely playing against you today (same for your skills, some of them solved problems that the current model isn't facing anymore)
breaking down what you built to rebuild it better is how humans have always improved, and models happen to evolve exactly the same way

and once this is done... I plan to complete as much as I can that weekend. And from there on, continue to do at least one task per day during the next week.
This definitely helped me make more progress... beyond the first weekend. 😅
