Cardiac Amyloidosis Diagnosis
Cardiac amyloidosis is a restrictive cardiomyopathy caused by extracellular deposition of misfolded amyloid proteins in the myocardium, leading to stiffening and heart failure.
Key Diagnostic Steps
1️⃣ Suspect in HFpEF with wall thickening, low voltage ECG, elevated troponin/BNP, or unexplained heart failure.
2️⃣ Screen for monoclonal protein:
🔹Serum-free light chains
🔹Serum & urine immunofixation
3️⃣ Bone scintigraphy (PYP, DPD, HMDP) is useful if no monoclonal protein is found.
4️⃣ Biopsy (fat pad or endomyocardial) with Congo red staining & mass spectrometry if needed.
TTR gene testing helps distinguish wild-type (ATTRwt) vs variant (ATTRv) amyloidosis.
