If you have food or liquid in your stomach, it can cause wahala when they give you anesthesia.
Normally, when you sleep naturally, your body still protects your throat, but when anesthesia knocks you out, those reflexes go off duty.
If your stomach still has food or water in it, your stomach content can come back up and enter your lungs (we call that aspiration). Once that happens, e fit cause serious problems like choking, pneumonia, or even death (I have seen a case where a mother snuck food in for her child hours before surgery and the child aspirated during the procedure. It took so much effort to bring that kid back)
That's why doctors always say, "No food or drink after midnight before surgery." It's just to make sure your stomach is empty, so nothing from your stomach can cause trouble you're asleep on the operating table.
In short: An empty stomach = safer surgery.
