@TPP_MD Absolutely. Ranted to my wife about this recently. But frankly the incentive just isn't there. Give up your young age to learn and then work a hard job. As compensation you were expected to be able to make enough to have a good lifestyle. Isn't worth it.

@YannickBuccella I had a lady come in for uterine cancer. Went to Mexico for intravaginal Ozone therapy, hyperbarics. Everything. Was told her cancer was cured. Came to me for abd pain. Had metastatic disease everywhere. Sad to tell them that there last year of treatments was useless.

@jimiuorio Hard disagree. Unless you completely change the exam. It's relatively easy to memorize just for a test. The test is designed under the assumption of schooling. You would need to make a separate exam designed to avoid typical test taking strategies, ie free text.

@owenbroadcast I deal with this all the time. I see CPS do nothing in tragic cases (sex abuse/drugs/neglect) but come barreling in on a freak fracture, plausibly explained. Overwhelmed and needs serious restructuring.
@CoffeeBlackMD We in the ED are always adapting and flexing but never breaking. We had a tent and trailer system set up for three+ years. During the peak we used hallways, waiting rooms, and even the lounge. Strangely enough it never felt completely overwhelming.

@DrDiGiorgio This is def an issue I face with trainees. No one wants to talk anymore. Send a message, which essentially is a rewritten h&p, over secure chat. Also given cultural demands, one thinks twice about calling someone for consult. But sending a text seems more innocent.
