@CritCareReviews I always find the concept of ‘I:E’ ratios more pertinent to ancient ventilators with cogwheels and levers, than to lungs which require a minimum expiratory time, (Te) to empty sufficiently. Its more useful to think of Te if the rate is being increased in the hope of clearing CO2

@CritCareReviews All very clever, but could confuse a trainee trying to work out what’s going on in the middle of the night. Unless the sodium is rising at an alarming rate, it’s probably not DI. Unnecessary desmopressin administration can be a disaster.
It’s so very easy to create a non-existent “thing” with a catchy name and then use it to show how much you know and to wield power over others. These ‘sound bite’ terms are difficult to disprove and patients are harmed. Sadly this is how we do medicine.
I remember some radiologists asking for my GMC number and asking me to type on the request “I take responsibility if contrast causes AKI” as a F2, to get a contrast CT scan done.
It’s taken a long time despite the evidence being there for years.
Are we risking that mythical beast that is(n't) contrast nephropathy? Or do we just need to make the diagnosis and treat the problem causing the real risk to the kidney(s). A tough call? I think not. https://t.co/uAZMR2O07H
