@eFNShelp my guess would be a mis-configuration of your webserver... should take an hour at most to find and fix and I understand it's been broken for months. I'd be happy to help out

@eFNShelp the problem is not with your form validation javascript code... since it is checking for .jpg files and the error message being thrown isn't the same.
Your problem must be server-side... Is the whole website broken? Do you need my help to fix it?

@eFNShelp I am unable to submit an application. Your form requests documents to be uploaded, but does not specify what format... I've tried .jpg, .pdf... and I get an error that the file formats are not supported and then the form is wiped clear! Please help!!
