Being brutally honest about the state of Pakistan’s middle class: one medical emergency and you’re broke. Lose your job and you’re on the street. This was never normal, not that I’ve known in my lifetime.

A medical doctor called me today asking help for concessions on his own child’s delivery at a private clinic in Islamabad. Professionals here live in debt, spend years just trying to pay it off, get harassed by debt collectors, and some don’t survive the weight of it.

This misery goes completely unheard while the country celebrates national milestones and international wins. Progress means nothing if the people living through it are quietly breaking.

Big victory for Pakistan in The Permanent Court of Arbitration, The Hague. Judges from USA, Belgium, Australia and Jordan said that Indus Water Treaty remained fully in force, as India had no justification for ending or suspending the agreement.

Federal Minister for Health Mustafa Kamal is under intense criticism these days over the horrible PIMS tragedy, and that criticism is fully understandable.

Politics aside, I am sticking my neck out to defend him. He is neither a doctor nor a public health professional, yet he has very quickly understood the real problems facing Pakistan’s health system. Listen carefully to what he is saying.

He has shown moral courage by openly accepting responsibility (I believe he was stopped by the PM from resigning). He is acknowledging how deeply the health system has rotted from within. He is pointing to political pressures in appointments and openly saying that this is a collective failure of successive governments - a governance issue that must be addressed both upstream and at the facility level.

Most importantly, he has been consistently drawing attention to the quiet deaths of nearly 400,000 under 5 children because of inadequate and suboptimal healthcare across the country i.e. more than 1,000 children every day. This is the real issue we need to tackle. And he knows how complex and difficult it is.

Having sat in this saddle myself, I fully empathise with him, however much he is under fire these days. At least he is showing courage. And, importantly, he is telling the truth.

Interim Report into PIMS tragedy finds

1. No water sprinkler system or fire alarm system was found in the ward where the babies were housed

2. No separate protocols or SOPs were in place to deal with fires - PIMS has 13 SOPs/protocols but there was no separate SOPs related to fires safety - two sub sections had references to fire safety

3. In June 2026 a fire broke out in the PIMS nursing hotel but nothing was done in its wake - not even a meeting was held to decide on future protocols / SOPs

4. No supervisor was present in the ward at the time of the fire - one house officer, two nurses and a security guard

5. The neonatal ward was found to be in violation of all WHO guidelines and requirements for such a facility

6. PIMS has no dedicated official to deal with emergency issues and matters such as caused by a fire

7. PIMS has no dedicated SOPs to deal with emergencies and in particular those related to fires

حیرت اس بات پر ہوئ وزیر اعظم اپنی ٹیم کو مبارکباد پیش کر رہے تھے۰ میرے مطابق انہیں قوم سے معافی طلب کرنے کی ضرورت ہے کے کس بدترین طریقے سے ملک کی معیشت کا حال کیا۰ ہمارے میڈیا کو بھی سلام جو آج اس کی تصوریر کشی کرنے کی جگہ حکومت کی بڑی کامیابی دکھا کر پیش کر رہے ہیں
