Dear Kitui's Daughter,

I love it when people ask intelligent questions, for knowledge and understanding. Thank you.

This is a beautiful question, and it arrives softly, the way trutth often does, curious, intelligent, refusing to be hurried. And it deserves an answer that is careful, and warm, and faithful to science, and also unafraid of nuance.

So let me answer you as a gynae doctor, and also as a woman who believes that medicine should explain, not intimidate, and should invite us closer, not push us away.
'How can people without an endometrium get endometriosis?'

First, let us clear the fog, gently.
Endometriosis is not the endometrium. It is endometrium-like tissue, similar in behaviour, and in hormone receptors, and in its stubborn response to estrogen, but it is not the same structure, and it is not always born in the uterus at all. Both Royal and American college (RCOG and ACOG) are very clear on this distinction. And that difference, quiet as it seems, changes everything.

For many years, medicine told a single story. It said: retrograde menstruation. Blood flows backwards through the tubes, cells implant, disease begins. And yes, this happens, and yes, it plays a role. But here is the inconvenient truth...one both RCOG and ACOG acknowledge without apology: up to ninety percent of menstruating women experience retrograde menstruation, yet only about ten percent develop endometriosis. So the old story, while neat, is incomplete. And incompleteness is often where the most important questions live. Modern medicine now listens to several stories at once.

One of them is coelomic metaplasia, the understanding that cells lining the pelvis, and the peritoneum, and even the pleura remember something ancient about themselves. Embryologically, they can transform. Under the influence of hormones, and inflammation, and genetics, they can become endometrial-like tissue. Which means this: you do not need a uterus, and you do not need an endometrium, and you do not even need to menstruate. The tissue can become endometriosis anyway. This is not speculation; it is embedded in RCOG guidance and echoed by ACOG.

Another story is older still, and quieter: embryologic remnants. During fetal life, Müllerian tissue migrates, and differentiates, and sometimes leaves cells behind small, silent witnesses. Years later, under estrogenic stimulation or immune imbalance, these cells awaken. This helps us understand endometriosis in women with MRKH syndrome, and in those with androgen insensitivity syndrome, and in pre-menarchal girls, and in people long after hysterectomy. It explains, too, those rare cases that unsettle social media and spark disbelief, yes, even cases in men.

ACOG also speaks of stem cells and immunity. Bone-marrow–derived stem cells can differentiate into endometrial-like tissue, and a compromised immune system may fail to clear cells that do not belong where they are. So the body does not simply create endometriosis; sometimes, it fails to stop it. And failure, in biology, is often less dramatic than we expect, and far more human.

AND TO MEN!

And what of men, and the TikTok whispers? There are documented, peer-reviewed cases of endometriosis in men, exceedingly rare, but real. They are usually linked to high estrogen exposure, as in prostate cancer treatment, or to the presence of rudimentary Müllerian tissue, or to embryologic remnants responding, at last, to hormones. These cases do not suggest menstruation. They do not suggest a hidden uterus. They simply confirm what medicine has been learning, slowly and humbly: endometriosis is a disease of cells, and hormones, and immunity, not merely of periods.

So let us answer the question plainly, and without fear.
People without an endometrium can develop endometriosis because endometriosis does not require an endometrium to exist. It requires susceptible cells, and estrogenic stimulation, and immune permissiveness, and time. That is all.

And my final word, because I really enjoyed responding to this. This is a postgraduate question you just asked😃👏👏👏👏👏👏... especially for women watching, and waiting, and sometimes doubting themselves. RCOG and ACOG are clear: endometriosis is real, and biological, and too often delayed in diagnosis. It is not caused by weakness. It is not imagined. And it is not invalidated by its complexity. Medicine is allowed to evolve. And questions like yours, gentle, insistent, brave are how it does.

Hey, so I paid for blue checkmark solely so I could talk about this and hopefully spread more awareness of why this SHOULDN'T HAVE HAPPENED, and to hopefully help my friends stay safe.

I work at cons.

(DISCLAIMER, I DO NOT REPRESENT THE CON I WORK FOR OR SPEAK FOR THEM IN ANY WAY. I am speaking solely from a place of experience, and what I have observed as both a con attendee and a con visitor for over 15 years.)

Every year, I work for a four day con with thousands of attendees and celebrity guests. This isn't to brag or try to get clout - I'm stating this to explain I have experience with this. One year, we found out a major Youtuber had quietly come to the con and had been spotted by his attendees. FOR HIS SAFETY, we immediately had a detail of plain clothed members of staff dispatched to help protect him and also keep people from crowding him or creating incidents that would cause a fire hazard due to people blocking pathways or exits.

For meet and greets, signings, photo ops, we use gaf tape, barriers, multiple bodies, and whenever possible, an area with limited access so we can keep track of who's coming and going, and there is ALWAYS multiple members of our staff alongside their staff. Even 'smaller creators' we make sure that they are in a position to be well taken care of. We also have constant communication with our team at all times.

This should have NEVER happened. There should have been- at MINIMUM- multiple rope barriers and several twitch con or convention staff nearby to direct people where the line was and to keep people moving and from approaching her like this. In some of the videos, I can see ONE rope barrier. That's it.

With Emiru already having been in danger earlier this year at a stalking case (the incident with Valkyrae and Cinna) and Emiru raising concern over her favorite and preferred bodyguard being banned after a similar incident, Twitchcon staff owed her safety, more than one singular rope and more than one guard. There should have been multiple ropes so you see someone trying to duck through before they get in. The high fabric barriers, multiple staff patrolling (both security and regular staff) to look for suspicious behavior, and handlers who, yes, stand in the way of onlookers taking pictures.

Do they ruin the shots? Yes. But you can have them out of the way for the fans in line while still creating a safe environment for Emiru. Onlookers don't need a perfect shot without guards, and that would create safety for Emiru.

I went to Comic-con as an attendee, and every meet and greet or signing I saw, had plain clothed staff leaning on the table, and uniformed staff present at the lines. The rope lines were zigzagged so it would delay anyone trying to run in.

Twitchcon, you failed Emiru. You failed your female creators and female attendees. In just a few hours, you've shown parasocial fans that you've created an environment where this is possible. While the public decries this and demands reform, there are those who are watching this and seeing that this is possible, and may be planning to do similar or WORSE to other female streamers before the con ends.

To my female/femme presenting friends at Twitchcon - Please be safe. Be aware of your surroundings, and if something seems off, trust your gut. Male/masculine/buffer friends, watch out for the smaller people.

Demand refunds, demand change, be vocal about situations like this. This isn't overreacting. Look up stories like Christina Grimmie. That happened in 2016. That wasn't that long ago.

Always protect your drinks, travel in groups. DO NOT SAY YOUR HOTEL ROOM NUMBER OUT LOUD. If you suspect you're being followed, tell your hotel staff.

When the con gives you an option for a survey, tank the survey and explain why. Do not be quiet about this. Be loud. Use your voice.

@crownpaw @dreamsofskies For reference, I had 3 LLs over ~4 years during this time. One was super short term (a couple of months), but the other two I had split between the rest of the time on normal leases with the two dogs on them. Paid a 1 time deposit for the dogs, and that was it. Anyway, good luck!

@crownpaw @dreamsofskies In my experience, you really should only have to worry about mentioning the dog & cat. I had 3 snakes and 2 bearded dragons in addition to my two (known to each of my LLs) dogs. None of LLs cared when they inevitably found out (why does the AC always die in the summer, lol?) 1/2
