QL muscle is anteriorly covered by any thorao lumber fascia not by fascia transversalis . Fascia transversalis is inner most fascia.

So it impossible to avoid phrenic with intersclene block.
Drug spread is also cephalocaudal .

Does this IP bursa play an important role in drug spread? In one article it was mentioned that quadriceps weakness is due to rupture of bursa and this bursa prevents drug spread towards obturator nerve. Thanks

In this lady QL Injection was given just at the tip of transverse process. Block was very effective on POD1.
