Hopefully this thrashes all the accusations. A neighbouring minister is praising the previous DMK Govt in terms of luring investors. TN saw all around industrialisation. Investments flourished across sectors & the state during 2021-25...🙂
At the @IndianExpress Adda, I spoke about how politics can be unforgiving. We competed fiercely with the previous DMK government in Tamil Nadu for investments - won some, lost some.
But I never heard an investor complain of being asked for money by the government. Credit where it's due: the DMK government ran a clean and professional investment process and that deserves appreciation.
"Doctor, I haven't passed urine normally in 3 years." That is not something, a 24 yr old lady usually relates to a Urologist. Ananya (name changed), 24 yrs, had visited 11 doctors. Her story sounded bizarre. She avoided long bus journeys. She never attended a wedding without knowing where the nearest hospital was. Every outing carried the fear that she might suddenly be unable to urinate.
Every few weeks, she would suddenly stop passing urine. Not difficulty, or poor flow. Complete inability. She would rush to an Emergency Department. A catheter would drain 800-1200 mL of urine. Relief. Then the cycle would repeat.
Urine cultures? Negative.
Ultrasound? Normal.
Kidneys? Normal.
MRI Spine? Normal.
No neurological disease.
No urethral stricture.
No pelvic mass.
Nothing.
One dr suggested anxiety. Another felt she was "holding urine voluntarily." A third prescribed antidepressants. Her family had started wondering if she had a mental problem. When she arrived at my desk, she carried a thick folder of investigations. Of course, the diagnosis wasn't inside it.
I started with a detailed history. The first episode had occurred a few months after recovery from a severe viral illness. Since then, her bladder seemed to fill normally. The problem was emptying it.Urodynamic study revealed a large, healthy bladder generating adequate pressure.But the outlet simply refused to relax.
EMG of the external urethral sphincter showed an abnormal pattern. I made my diagnosis- Fowler's Syndrome.Her sphincter filled the bladder fine. It just refused to let go. A condition we call Fowler's Syndrome, rare, real, and almost never caught. A condition affecting young women where the urethral sphincter fails to relax appropriately, producing chronic urinary retention despite the absence of any anatomical obstruction.
Her kidneys were normal.Her bladder was normal.Her scans were normal. Yet she could not perform one of the most basic human functions. We proceeded with Sacral Neuromodulation, with our neurosurgery colleagues. Weeks later, she returned.
No more catheter. No emergency visits. No fear of being stranded somewhere unable to pass urine. She smiled,& said: "Doctor, I never thought urinating would become something I could be immensely grateful for."

Any warmth, optimism and positivity that you have towards Chennai evaporates the moment you try catching a cab after stepping out of the airport
Because unlike most cities where you get out, open an app, book a cab and go, in Chennai you have to
Haul your luggage in the enervating heat to an overcrowded buggy stand.
Then wait for an buggy which takes an eternity to arrive.
Into which you are then stuffed like a sardine in a can and driven for 20 mins to a god forsaken "mall"
Where you again have to compete with a batallion of tired and hungry travellers and their luggage to get onto an elevator
Which will take you 3 floors up to a taxi stand where you hope there is cab available
Which will then drive you down those very three floors to take you into the city
I seriously cannot comprehend how the supposedly knowledgeable Chennai people messed up something as simple as an Airport cab pick up
All of Chennai's aspirations to be a megapolis dies in that gap between the airport arrival and that cab pickup station
